All of the men in my family are above six feet and are strong and quick. Strong runners and strong in most athletic activities.

3. What are your interests and hobbies?

I have a passion for anything active. It keeps my mind and body clear, moving, and changing. Trail running, street running, hatha yoga, indoor rock climbing, mountain bike riding, squash, weight training, extreme hiking, kick boxing, and meditation. I do a combination of all of the above to stay healthy and alive.

I love the arts, including (1) visual arts: I recently took three painting classes and have a portfolio of water colors; (2) theater arts: I participated in plays in high school, took acting classes, and had a 5-year contract with a modeling/acting agency after high school; (3) musical arts: I participated in church choir and high school concert choir, and now love music and listen to cable music channel 471 whenever I’m at home.

Comments Following an Interview with the OHSU Fertility Consultants Clinic Counselor:

Donor #9859 is a thirty-two-year-old, single, Caucasian man that learned about sperm donation from an advertisement in a newspaper. Donor works full-time as a fitness instructor/personal trainer. He is a current college student seeking a four-year degree in Psychology. Donor has worked as an assistant physical therapist, a certified nurse assistant, and a certified medical assistant. He states his motivation to be a donor is financial (to pay for school), but throughout the process of participation in the donor program he thought more about what he was actually doing to help other families and feels comfortable passing on his gene pool anonymously.

Donor's mother works for non-profit organizations in the field of aging. His father retired at age 52 and was in real estate. Donor has no history of mental illness in the family of origin and no counseling history. There is no history of alcoholism in his family. Donor is a non-drinker, non-smoker, and does not use recreational drugs. He has no legal or financial difficulties. His hobbies include athletic pursuits, and he is very active. He does hatha yoga, trail running, rollerblading, squash, racquetball, and works out daily. He is very fit and trim. He makes it a priority to stay fit in his life.

He is very friendly and warm with a nice smile, healthy complexion, fine features, and a positive attitude about life. He meets all the ASRM psychological criteria for being a donor, and I recommend him.

Summary of the Taylor-Johnson Temperament Analysis:


The Taylor-Johnson Temperament Analysis was administered to Donor #9859, and he scored in the Excellent range in eight out of nine categories. Donor is a thirty-one-year-old male, and this analysis was based on his age group. The ninth score was in the Acceptable range, which overall represents a very balanced personality type.

The first Excellent score was in the personality characteristic of Composed versus Nervous. This donor was rated in the middle of Excellent which indicates a tranquil and possibly relaxed style, or someone that tends to be calm in all kinds of situations. They are not high-strung and would like not be upset in tense situations such as high traffic or time pressure. People that are nervous tend to more likely be smokers, overeaters, nail-biters or use drugs or alcohol for relaxation.

The second Excellent score is Light-Hearted versus Depressive. This is very compatible with the first score of Composed. They go together because these kinds of traits represent a person that is optimistic, cheerful and/or happy in their disposition. They have a sense of well-being in contrast to looking at life with apathy or pessimism.

The third area of measurement also in the Excellent range was Active-Social versus Quiet. This donor was about off the chart for Active-Social meaning he has many outside interests and enjoys pursuing them. He would likely be socially involved, energetic, and a "go-getter" in life. This measurement looks at participation in community, club, or group events or activities, making new friends, and a desire to be with friends and people. There is a need for companionship and a pleasure in it.

The fourth area of measurement, scoring again in the Excellent range, was Expressive-Responsive versus Inhibited. This goes hand in hand with the previous score because this is a measurement of a person's ability to be spontaneous, affectionate, and/or demonstrative. This is the kind of person that shows warmth and expresses friendliness to others. They act with affection towards their families, express tenderness, and are also able to share both joys and sorrows.

The fifth Excellent measurement was in Sympathetic versus Indifferent. This is defined by traits that express compassion, understanding, and empathy. They are likely to be aware of others' needs for support or encouragement, and they enjoy children, the elderly, and/or animals.

The sixth Excellent score of donor #9859 was in Objective versus Subjective. People that are objective are not self-absorbed but are reasonable, fair-minded, logical and tend to be impartial. They are not self-conscious or overly sensitive.

1. In general, please describe your childhood.

My parents and I lived in the same house for fifteen years. We did not move until after high school. Very stable environment. A great neighborhood with other kids and families, so we had lots of block parties and barbeques. Safe neighborhood for bike riding and outdoor games. There were some older kids who would entertain us younger kids with various games and activities.

I took violin and piano lessons as a child, and to this day I love music.

2. Were there any family members other than your parents and siblings with whom you regularly had contact?

I saw all my cousins on a regular basis. We were pretty much laughing and playing together from the moment we saw each other. We had a blast. Very excited for holidays or birthdays. My cousin and I were two days apart in age, so our birthday celebrations were often together.

3. As a child, what was/were your favorite: (Please include a reason, or why, if you can.)

6. Are there any "new" technologies you remember becoming available during your childhood which now are commonplace in our society, or which have now been replaced by a newer technology?

Cable television. VCRs. We seemed to be the first in our neighborhood to have one, and now of course we have DVDs and other such items.

7. Did you enjoy school as a child? Do you have any memories of teachers that influenced you, or that you especially liked (or disliked)?

I was very excited to start first grade. My teacher was excellent in keeping students busy and learning. I did enjoy school; however, I was disappointed that I did not learn everything the first day.
